Indications

Ortho Evra is a hormonal contraceptive transdermal patch used to prevent the user from becoming pregnant. It contains the same hormones, estrogen and progestin, that many combination birth control pills do. Like birth control pills, it should not be used for the prevention of sexually transmitted infections such as HIV (the virus that causes AIDS) because it provides no protection against infection.

Use

The Ortho Evra patch is applied to the skin weekly for 21 days, three weeks out of each month. During the fourth week of the month, no patch is used, during which the patient will menstruate. New patches should be applied on the same day of each week. The area of the skin where the patch will be applied should be clean and dry. The patch should be applied to areas on the buttocks, the outer portion of the upper arm, the torso or abdomen where it will not be susceptible to friction from tight clothing. The patch should not be used on the breasts. To prevent irritation to the skin the patch should be applied each week to a different area of the skin and should not be used on skin that is inflamed, broken, or red. If the skin underneath or near a patch becomes irritated or inflamed, the patch can be removed. Use a new patch on a different area until the next day to change the patch. Powders, makeup, oils and oil-based lotions and creams should not be used on or near the area where the patch is applied to avoid loosening the patch. Do not wear more than one patch at a time.

Precautions

Patients should see a doctor regularly during their use of the patch if they have or have had diabetes; cardiovascular diseases such as high blood pressure, high cholesterol, angina pectoris, stroke, heart attack, or clotting disorder; breast or reproductive cancer; liver tumors (benign or malignant) or jaundice. Having one or more of these conditions significantly increases the chances that a patient will experience complications from the use of the Ortho Evra patch.
Ortho Evra is known to cause birth defects during gestation, so it should not be used while a patient is pregnant. Ortho Evra decreases breast milk production. The patch should not be used while breastfeeding.

Missed Dose

If the first patch during the month is forgotten, pregnancy could occur and back-up contraception should be used for the first week of the cycle to prevent pregnancy. Apply the patch as soon as it is remembered and use that day as the new day to change the patch each week. If the patch is forgotten for one or two days during week two or three, apply a new patch as soon as it is remembered. A back-up method of birth control is not needed. If it is forgotten for more than two days, a new four-week cycle should be started. The day that the patch is remembered should be the new day that the patch is changed weekly. If during week four removal of the patch is forgotten, remove it as soon as it is remembered and start a new patch cycle on the normal day after the last day of your cycle.

Contraindications (Side Effects)

Ortho Evra carries the risk of many of the same side effects as some birth control pills. These side effects include discharge from the vagina, changes in breast size, breast pain and tenderness, changes in the menstrual period, headache, nausea, cramping of the abdomen and bloating.

Storage

Ortho Evra patches should be stored at 77 degrees Fahrenheit but may be kept briefly at temperatures between 59 and 86 degrees. Patches should be kept in their pouches and applied immediately after the pouch is opened. Used patches have some remaining hormones and should be treated carefully. To dispose of a used patch, fold the patch in half, sticky sides together, and place it in a garbage can. Do not flush used patched in a toilet.

Overdose

Ortho Evra releases hormones at a regular rate, so one patch is unlikely to cause overdose. Using more than one patch may cause vaginal bleeding, nausea, and vomiting. Do not use more than one patch. Children ingesting hormones may experience severe illness.
